A 3D nude Samus model was in the N64 version of Super Smash Bros.
Oh, my...
Oddly enough for a Nintendo game, a three-dimensional, texture-less model of what appears to be a suitless, naked Samus was actually viewable in the HAL Laboratories-developed game Super Smash Bros. for the Nintendo 64.

Directly after Samus is hit by a powerful attack (a fully-charged Samus beam shot, for example), she appears to flash through different colours very quickly for a short period of time. If the game is paused at the exact right moment, it is shown that one of these flashes turns out to be a naked Samus.

Granted, the N64's processing power isn't exactly impressive in contrast with today's standard, and so the 3D model is deprived of a big enough number of polygons. Nevertheless, it's funny to think that the game could have received an M rating instead of an E, had the ESRB spotted this.
